IT change control form
Overview
This easy-to-use change control form will help you better manage IT change requests.
Everybody benefits from proper change control. Beneficiaries range from those on the Help Desk, to IT managers, business clients, and even the implementer himself. By having a proper change control process, the scope of change is managed better. Those not involved directly in the change (such as the Help Desk) will have a greater understanding of why a system may not be working. Rather than spinning their wheels diagnosing a problem, a quick referral to the previous nights change control document/calendar might explain everything. For IT managers this is particularly important from an accountability perspective. Clients also greatly benefit form knowing about pending changes because it gives them the ability to schedule their work around such changes or to even veto them during critical business cycles.
Documenting all change requests also helps avoid the inevitable scope creep, sets expectations for stakeholders, and protects the implementer from finger pointing if unrelated systems are adversely affected.
Our customizable Change Control Form is easy to use and implement, searchable, and accessible by anyone with Word. The form includes the following sections:- Details
- Schedule
- Resources
- Risk Assessment
- Risk Mitigation
- Rollback Plan/Test Plan
- Approval
